How far in advance do you need to schedule your appointment?
We would like you to schedule your appointment as soon as you know. However, a three-day notice is acceptable but not guaranteed. We can also accept some same-day call-in appointments.
What insurance covers wheelchair services?
Medicaid—ModivCare is the broker for Medicaid. Patients must contact them at 866-445-9954 to schedule an appointment and request their preferred provider. Once you have completed the following steps, contact our office with the trip number.
What insurance do we accept?
We accept all major insurances. However, the plan must cover transportation.
What level of services do we offer?
Ambulatory- Patients who can walk without assistance.
Wheelchair- Patients who are wheelchair-bound and require a lift.
Stretcher- Bedridden Patients.
